How many alcoholic drinks per week may a person on mtx have?
Is it relatively safe to have 1 or 2 alcoholic drinks per week if I give myself 12.5 mg of methotrexate per week?

The official "party line" is that all alcoholic beverages are to be absolutely avoided due to the increased risk of liver toxicity. That being said, the degree of risk that 1-2 drinks per week poses has never truly been established.
Realistically, occasional, low-to-moderate alcohol intake is probably something many patients taking methotrexate are doing already. My recommendation would be to try to limit the amount of alcohol that you drink (without putting a specific number of drinks per week), recognizing that rare/occasional usage is probably of limited consequence, but with each additional drink there is an additional risk of liver damage.

My tip is this: Do all the same habits, same beverage container, same routine, just CHANGE what's in the glass/bottle/tumbler etc. I used to love my red wine. I had 2 to 3 glasses each evening while I chatted with my husband and cooked dinner. When I started on MTX my rheumatologist said no more than 3 glasses per week. I decided that I'd be better off quitting altogether and only having a little wine on very special occasions, holidays, family get together, special celebration, that kind of thing. I used my favorite wine glass and filled it with Pelogrino. I kept the bottle of the fancy water on the counter where my wine bottle would have been. I drank the water just as if were the wine. I found that it was WAY easier to deal with the cravings when I did this. These days my cravings are long gone and my liver is happy. I have friends that this method worked for, hope it works for you. :)
I've been on MTX for 20+ years and have never had a rheumatologist tell me not to drink. I don't drink heavily but I do have a glass of wine or two most nights. That said, I've always had regular blood work done every 8-12 weeks to check for liver toxicity. I've always been told that that is the way around this issue.
It can be done a lot of damage in your body and you can feel the health issues after sometimes. The academic paper writers share why it has happened and why you don't go for it.
